This Portal is for the exclusive use of hospital clinicians to send discharge letters, clinic letters, or general correspondence to a patient's GP.

Any patients or unauthorised users should not expect any acknowledgement or response to an entry using this portal.

Please read through the list below carefully.

If you intend to requested any of these tasks from the GP, you may be in breach of your Trust's contract, professional obligations to your patients, and your governing body's good practice guidelines:

  • A FIT note required directly for a condition managed by you or your team.
  • The prescribing of a medication that the patient should be discharged with (1 week supply), initiating a RED or AMBER drug or ongoing prescribing of an AMBER drug without the consent of the GP who has agreed to the transfer of care under a shared care agreement. For patients treated privately, all medications should be issued by the private clinician.
  • A request to chase the results undertaken by you or your team.
  • A request to undertake investigations on behalf of you or your team.
  • A request to make an onward or re-referral for the same condition within your own/ same Trust.

Our practice reserves the right to not fulfil your request and to direct the patients back to your services.

Although the GP would meet any clinically urgent requests, anything else may delay or hinder patient care.
